TEST OUR SYSTEM

Please follow these few simple steps to access our secure website and view some sample test results for yourself, always available on any internet-capable computer in the world.

  1. At the left, under "Login" please enter "QLS" into both the username and password fields. You've now accessed our HIPAA compliant secure website!
  2. In the left column, click on "Result Inquiry".
  3. Please enter "test" into both the Last Name and First Name fields and click "Start Search".
  4. Click on any of the available "Test" links. This will access a complete blood workup.
  5. Click "Cumulative Report" at the top of the screen to see a side-by-side comparison of up to 6 former workups, allowing you to instantly see any trends in that patient's blood history.
  6. If you would like to print the results, simply click "Print" at the top of your screen.

Shipping Instructions

To insure the most rapid turn-around times and to preserve the integrity of the specimen, please follow the guidelines listed below. Please call Client Services at 718-646-5100 if you have any questions regarding test ordering, specimen requirements, or supplies listed below.

Federal Regulations

U.S. Mail, Federal Express and Airborne Express regulations require specimen packaging to include the following: 
 

  • Watertight primary receptacle.
  • Watertight secondary packaging with biohazard labeling.
  • Absorbent material (desiccant) placed between the primary receptacle and the secondary packaging.
  • Sturdy outside packaging constructed of corrugated fiberboard (cardboard), wood, metal or plastic.

Courier services are designed to facilitate efficient delivery of patient care by ensuring prompt specimen pickup and turn-around times. Quality Laboratory Service employs own trained, uniformed couriers to guarantee specimen integrity. STAT pickups are available for urgent testing needs. QLS utilizes state of the are information systems to schedule and track courier pick-up and delivery. We can accommodate virtually any specific pickup requirements. For more information, call us @ 718-646-5100. Quality Laboratory Service provides all necessary supplies and containers for laboratory diagnostic services at no charge. This includes pre-paid mailing containers for clients who wish to mail their specimens to us.
 
Quality Laboratory Service provides home visits for laboratory testing to those patients who are medically unable to go to a patient service center or the doctor's office. We service over 12,000 patient lab encounters annually including over 800 home care visits.
 
A highly trained phlebotomy technician, visits the home. In most cases the technician will call the patient the evening before the visit to verify an approximate time of arrival.
 
Our Home Care department provides you with a single point of contact for scheduling and follow-up. Simply call the Quality Home Care Department directly at 718-646-5100. Hours are Monday-Friday, 9:00AM to 5:00PM.
