CPT Code: 83021

Notes: CPT Code = 83021 (83021>1=83021). The initial HPLC screen will quantify Hb-A, Hb-F, Hb-A2, and other hemoglobin variants. Presumed Hb-S by HPLC will undergo confirmation solubility testing for an additional fee. If reflexed, add CPT code 85660. The presence of Hb-variants by HPLC will reflex to electrophoresis, for an additional fee, where other phenotypes can be identified and confirmed. Highly abnormal patterns will be reviewed and interpreted by a pathologist. If reflexed to electrophoresis, add CPT code 83020.
