Clinical utility

Differential diagnosis of primary hypothyroidism and hyperthyroidism from normal; differential diagnosis of primary hypothyroidism from secondary and tertiary hypothyroidism; thyroid function test (TSH is high in primary hypothyroidism, low in hyperthyroidism); evaluate hypothyroid patients receiving replacement doses of various thyroid hormone preparations; detect hypopituitarism; follow up on low T4 newborn screen results.

Notes: 1) A suppressed TSH (< 0.1 uU/mL) suggests a diagnosis of hyperthyroidism, elevated concentrations (> 7.0 uU/mL) suggest hypothyroidism. TSH is more sensitive than free T4 in detecting changes in thyroid function, both hyperthyroidism and hypothyroidism. Rarely, pituitary tumors can produce TSH, causing secondary hyperthyroidism, making free T4 necessary to clarify the result. 2) To obtain serial graph reports order test code #3250SR.
